Sidewalk or Curb - Repair at 121 Strathlea Co SW

Large crack in sidewalk. I have called 311 several times over the last 4 years to report this concern and I have never had any response. I am concerned that someone may trip and fall if they are not watching where they are going.
Nature of Concern: Cracked / Crumbling
Type of Sidewalk: Concrete
Specific Details: Large crack in side walk. Chunks of concrete are loose and neighbor hood children are pulling bits out.
Specific Location: Public Side walk at the east side of my drive way.
