Road goes from two lanes to one lane with no signage

Details of request: There is a sign showing that the two lines jog to the right but both continue, however what actually happens is that one lane just disappears. This has created many near accidents due to improper signage. The lanes also need repainting as they are adding to the confusion.

Specific location details: East bound on Country Hills Blvd just past Coventry Blvd.
